COMPOSITION Clotrimazole USP 1% w/v, Lidocaine Hydrochloride BP 2% w/v, Propylene Glycol USP q.s. 

INDICATIONS: Clotrimazole and lidocaine ear drops are indicated for the treatment of fungal infections of the outer wear (otitis externa) and middle ear (otomycoses) with pain (otalgia). 

CONTRAINDICATIONS: Clotrimazole and lidocaine ear drops are contraindicated in the following conditions: Hypersensitivity to the any ingredient of this formulation. Hypersensitivity to other azole derivatives. Viral infections of ear including varicella and herpes simplex infections. Perforated ear drum or tympanic membrane. In case of perforation of the tympanic membrane (due to potential ototoxicity), if in doubt, consult a doctor for otoscopic verification 

SPECIAL WARNINGS AND PRECAUTIONS FOR USE: This product is for optic use only. Clotrimazole and lidocaine ear drops should be discontinued, if there are signs of sensitivity to any of its ingredients. These ear drops should not be used for prolonged period of treatment. To keep the medicine as aseptic and germ-free as possible, do not touch the dropper to any surface (including the ear). Do not use in children under 12 years of age, except under the advice and supervision of a doctor when clearly indicated. Plain suitable antifungal or antibiotic ear drops for the treatment of uncomplicated Otomycosis or bacterial otitis externa in children respectively must be preferred over combination products. Keep this and all drugs out of the reach of children. In case of accidental ingestion, seek medical assistance. Optic chloramphenicol preparations should be avoided in patients with perforated ear drum because of a higher risk of ototoxicity. This product contains lidocaine; in cases of otitis media, preparations containing a local anesthetic agent can exert transient analgesic effect which may delay the diagnosis. Thus, the use of this product without medical advice is limited to 5 days in order not to confuse the diagnosis. Prescription of this product for children and infants should be cautious given the higher risk of systemic absorption in these patients. 

UNDESIRABLE EFFECTS: Clotrimazole: As the listed undesirable effects are based on spontaneous reports, assigning accurate frequency of occurrence f is not possible. Immune system disorders: allergic reaction (syncope, hypotension, dyspnea, urticaria). Skin and subcutaneous tissue disorders: blisters, pain, oedema, erythema, irritation, peeling / exfoliation, pruritus, rash, stinging / burning. Lidocaine: Generally, lidocaine ear drops do not produce adverse effects. However, penetration of the local anesthetic in the ear can cause symptoms such as dizziness, nausea and nystagmus. Irritant and allergic reactions in the auditory duct are unlikely but possible. In this case, stop the treatment. 

OVERDOSE: Clotrimazole: There is no specific antidote for overdose of clotrimazole. However, in the event of accidental oral ingestion, routine measures such as gastric lavage should be performed only if clinical symptoms of overdose become apparent (e.g., dizziness, nausea or vomiting). Gastric lavage should be carried out only if the airway can be protected adequately. Lidocaine Symptoms: Overdose of lidocaine may occur only in very young children due to too frequent instillations or accidental absorption. Despite its low bioavailability, accidental ingestion of lidocaine topical solution can lead to the symptoms which may include following: numbness of the tongue and oral cavity, swallowing difficulties, paresthesia, restlessness, vision confusion, disorientation, myasthenia, nervousness, tremors, headache, convulsions, central nervous system depression, dyspnea and tachycardia. Treatment of accidental ingestion: Immediate gastric lavage in case of accidental oral intake causing discomfort. Symptomatic Treatment IV administration of diazepam in hospital in case of seizures. Oxygen therapy and assisted ventilation if necessary.
